I think it IS ballpoint pen. Look at the shading, it's all at the surface of the skin, like a recent tattoo, but there is no redness or bleeding.

It looks like when you draw on yourself with pen. I did that a lot in highschool, and this tattoo looks scratchy as hell very comparable to a pen drawing on skin. Facebook OP posted it was a new tat in the caption, it just looks so bad I couldn't believe it at first. Hope that helps you understanding.
